So in the end, the dermatologist convinced herself that giving me a cream to apply to the spot would be a better treatment than the nitrogen freezing. I apply the cream 3 times a week for a month. She says it'll be like have a little, crusty cold sore for a month. I can live with that.

I realize that what I hadn't mentioned before was that my dad has a spot on his lip frozen off a year or two ago. He's not a person who complains about pain usually, but he said it hurt, a lot. It made eating & drinking very difficult, looked horrible, etc. It's possible that his dermatologist wasn't very experienced, but my suspicion is that they get so used to doing this procedure, which admittedly is no big deal most of the time, that they don't stop to think that the lips are more sensitive than the rest of the face.

Anyhow, it was nice to find out there was this option, though I'm not crazy about this doctor either, even though she seemed nice enough. Her English wasn't very good, but she used it volubly and it was hard for me to get a word in edgewise (which is why I said she talked herself into giving me the cream instead of doing the freezing. I didn't have much input). Whatever, I'll see how the cream works.

Oh! Possibly you're just stringing out the pain in smaller increments for longer? Sure seems like an alternative worth trying, however!
